Today I created a pre-training skills assessment survey using surveymonkey - ahead of doing some Interactive Whiteboard training for colleagues. It's here.

I cheated a little - there was another surveymonkey already done by someone using Activboards. Mine was for Smartboard. Hope that person didn't mind me using theirs as a model, I did have to change questions.
Now I just have to wait for some feedback from manager, then see if anyone actually fills it in!
I needed to do similar for VLE training (to happen in the same training session!) but took the easy route and edited a spreadsheet with conditional formatting that I used for general ICT initial assessment, to be emailed round. Yes, an interactive thing would have been more fun but sometimes you just don't have the time to play.
